The information contained in this application and screening form is correct to the best of my knowledge. I authorize any references or churches listed in this application and screening form to give you any information (including opinions) that they may have regarding my character and fitness for working with children. In consideration of the receipt and evaluation of this application and screening form by Community Covenant Church, I and my personal representatives, assigns, insures, heirs, executors, administrators, guardians, spouse and next of kin hereby releases, waives, discharges and covenants not to sue Community Covenant Church and its directors, elders, officers, employees, agents, members, volunteers as well as its successors, assigns, affiliates, subordinates, subsidiaries, and any other individual, church, youth organization, charity, employer, reference, or any other person or organization, including record custodians, both collectively and individually, herein referred to as “Releasees,” from any and all liability for damages of whatever kind or nature, whether caused by the negligence of Releasees or otherwise, which may at any time result to me, my heirs, my family, or any other person or organization on account of compliance or any attempts to comply, with this authorization. I waive any right I may have to inspect any information provided about me by any person or organization identified by me in this application and screening form.
Should I be allowed to work with children, I agree to be bound by the Articles of Incorporation, Bylaws and Policies of Community Covenant Church and to refrain from unscriptural conduct in the performance of my services on behalf of the church.
I further state that I HAVE CAREFULLY READ THE FOREGOING RELEASE AND KNOW THE CONTENTS THEREOF. I UNDERSTAND THAT THIS RELEASE IS A RELEASE OF ALL CLAIMS INCLUDING NEGLIGENCE OF RELEASEES AND I SIGN THIS RELEASE AS MY OWN FREE ACT. This is a legally binding agreement which I have read and understand.
